Lakeview Medical Center participates with most major insurance carriers. Participation may vary by provider. New patients should verify that their Lakeview physician participates in their insurance plan at the time they make their first appointment.

As a courtesy to our patients, we will file the necessary claim forms. Please remember that we cannot guarantee payment by your insurance company. Payment for the office visit is due at the time of service. Any remaining amount not covered by insurance is due upon receipt of the statement from Lakeview. It is important that you familiarize yourself with the exclusions and limitations of your insurance contract.
