To expedite the new patient registration process, please take a few moments to review the following information and complete any necessary forms before arrival:

• New Patient Information Forms.

• Lifetime Signature Agreement Form for Medicare patients.

  Lifetime Signature Agreement Form (Requires Adobe Acrobat Reader)

• Insurance Verification Form.

  Insurance Verification Form (Requires Adobe Acrobat Reader)

• Financial Agreement Form and Release of Information.

  Financial Agreement Form (Requires Adobe Acrobat Reader)

• Referral Letter of Understanding. (if required by insurance)
• List of your Current Medications. (Please bring to appointment)

Plan to arrive at least 30 minutes before your scheduled appointment time to allow Central Registration to complete the necessary paperwork.

In order to correctly file your insurance(s), please bring the following items with you:

• A picture ID (Driver's License, Military ID card)

• Primary and Secondary insurance cards(s)

• Valid referral if your insurance requires one.

• A co-pay if your insurance requires one. (we accept cash, check, Visa, Mastercard, and Discover cards).

For your convenience, you may pay any unpaid balances at the West Wing registration desk. There are two financial counselors available to assist you with payments or any questions you may have.
